Next stage of TransformLDN – gathering the views of service users
In October 2014 the "Declaration of the Foundations of Community Services" was launched. This was co-created by 1,000+ health and social care staff, who were engaged using our crowdsourcing methodology, to share their views on what excellent community services look like. The 4 Foundations of the declaration can be regarded as the key "ingredients" for good community services:
- Get Personal
- Make Boundaries Invisible
- Real Leaders, Happy Workers
- Focus on Outcomes

We are now working with NHS England on the next stage of this project to embed the Community Foundations into practice. We're running a series of face to face and online workshops with the people who use London's community services. The aim is to further to test, validate and spread these Foundations.
This project was devised, as the team behind the creation of the Declaration felt strongly that we need to get feedback on Our Declaration from the people that use Community Services to improve it and ensure it is relevant for everyone.
